Mission
Statement
The
Massachusetts CFIDS and FM Association is dedicated to
working and advocating for effective medical research and
treatments that will both alleviate patient suffering and
lead to a cure.
We provide
information and supportive services to help patients, their
families and loved ones cope more effectively with these
illnesses.
We work to
educate both health care providers and the general public,
so that CFIDS and FM will be recognized and understood as
real, serious and disabling physical
illnesses. |

Physician Referral Questionnaire
|
Dear CFIDS or FM patient,
family member, or other interested persons:
Please help us and other patients by taking a
little time to print out and then fill out our
physician referral questionnaire. Then send it
back to us by mail.
One of the most important services we at Mass
CFIDS/FM provide is patient referrals to
health care providers.
We need to update and expand our health care
provider data base. You can help us with this
by filling out the questionnaire.
You know the importance of good referrals to
doctors and other providers who have the
expertise to be able to diagnose and treat CFIDS
and FM.
We have a health care provider database obtained
from years of patient feedback. We need to
update and expand this list, so that we will have
more current and helpful referrals.
We ask you to take some time, when you are able,
to fill out the questionnaire and return it
to the address given on the form. If you need to
take a few sessions to do so, do it in
steps. It looks long, but if you pace yourself,
you can complete it. Also, if you can’t
answer every question and only partially fill out
the form, please do so. Every
questionnaire we receive back is valuable and will
help a number of other patients.
Your confidentiality will be guarded closely.
Remember, by filling out the questionnaire you are
helping other sick people.

and leave a message. We'll get back to you as soon as
possible (inquiries regarding health care provider
referral will be returned on Thursdays between the hours
of 2 and 4

CAN YOU
HELP US PUBLICIZE OUR WEBSITE BY DISTRIBUTING OUR
OUTREACH FLYER?
You can
print out our one-page flyer that tells people about
our organization, our services and our WEBSITE. We
would really appreciate it if you could make extra
copies and leave them at places where other patients
might pick them up, such as:
- Doctors
and other health care providers' offices
-- especially if they treat CFIDS or FM patients,
- Health food and Vitamin stores,
- Support groups,
- Community bulletin boards and libraries,
- Places of Worship
Be
creative. The idea is to put some flyers in places
patients are likely to go. Also, if you are in touch
with other patients via the internet, ask them to
print out the flyer for distribution.
